Bona-fide members of the following:
Assist with upper body strengthening, teach activities of daily living, and use of adaptive equipment.
Evaluate the patient with communication and swallowing deficit (for persons who had a stroke, accident, or laryngectomy). Design a program to rebuild function.
Social worker will assess and remedy social environment and emotional factors which relate to the patient's health problem and utilize appropriate family and community resources for adjustment and rehabilitation.
We do eligibility test for insurance coverage and process all claims. In cases where co-payment is indicated, you will be informed ahead of time. Assists in evaluating the patient's nutritional needs and planning for home dietary management. Counsels patient and/or family in the home when necessary. Charts nutritional services rendered for the patient and participates in the performance improvement program.
Assist with bathing, dressing, personal care, meal preparation and nursing care as directed by and under the supervision of a skilled nurse.

Services are given by either a registered nurse (RN) or a licensed vocational nurse (LVN). If you get services from a LVN, your care will be supervised by a RN. Home health nurses provide direct care and teach you and your caregivers about your care.
Design and implement exercise programs in the home to assist in regaining mobility and function. Train in proper use of special equipment to ensure safety with walking and transfers.
